Food Vendor Application

Please read all guidelines and requirements before submitting your application. Once received, it will be reviewed by the Labor Day Food Vendor Committee. If selected, you will be contacted for instructions on submitting a money order or cashier’s check securing your vendor spot. This year’s festival will be held Friday, Aug. 29, 2025, through Sunday, Aug. 31, 2025 at the Choctaw Nation Capitol Grounds, Tvshka Homma, Oklahoma.

APPLICATION GUIDELINES

  1. The deadline to submit applications is Friday, May 31, 2025.
  2. Applicants must submit photos of their food station, including inside and outside (storage areas, smoker, grill, sinks, hand wash station, cook station, prep station, holding station, etc.) with this application. (Required)
  3. Applicants must submit photos of their food items, menu and menu prices (if there is a specific menu for this event, please include them with the application. (Required)
  4. Applicants must submit a recent Health Inspection Report and their state Food Permit from the Department of Health.(Required)
  5. Applicants must submit their CDIB (Certificate of Degree of Indian Blood) if applicable.

The required information allows vendors to effectively showcase their business to the Food Vendor Selection Committee.
